Boccacio's has a handful of awards and is old world/school styled Italian food.

Vacarros is an italian pastry shop, you can grab a cannoli and eclair or some great cookies or something.  not something that will take too long. you could even get something to go!

I've tried about four different Italian restaurants in Little Italy and have been disappointed by them all. IMHO, Little Italy is way overrated and best avoided. The best Italian restaurant in Baltimore in my experience is actually on South Charles St. It's called Vespa.

Brass Elephant is definitely one of the top 3 or 4 best restaurants I've eat at in Baltimore, but I would not call it an Italian restaurant at all, although they do have a pasta dish or two on their menu.

Germano's is excellent.  Been there plenty of times with no disappointments.  Valeggia's is also good.

Not Italian, but McCormick & Schmick's seafood resteraunt located on Pier 5 on the Harbor is excellent as well.

As I said before, go to BOP instead.

Or better yet, impress her with a lovely extra special home cooked meal.
