I know you're joking but that film would never get made today with better acceptance of LGBT+ individuals. The whole "the antagonist went mad at the end and was driven to cross-dressing" plot point is wildly offensive alongside . . . all the rest of it.

Not to mention, right thinking people have been avoiding Ween and fighting Ween-representation for decades.

but still was somewhat ahead of it's time in terms of representation

still love when Harvey K is stranded on a island and has sex with pat and is like "Even though we had sex, I'm still not sure what sex pat is"

I think the "joke" is technically on those adjacent to Pat who are driven to anger/madness/etc because they are unable to process this person who does not easily slot into gender-normative appearance, but that is likely a distinction without a difference.
